Dear Parents and Guardians,

We are thrilled to welcome you to our school community for the upcoming academic year! As we prepare for a new and exciting journey of learning, growth, and discovery, we understand that effective communication and collaboration between home and school are essential for the success of our students.

This page serves as a centralized hub where you can access all the necessary forms and documents required for the school year. By completing these forms, you play a crucial role in ensuring that we have up-to-date information and can better support your child's individual needs and interests.

Here's a brief overview of the parent forms available:

  1. Academic Integrity Pledge: Reinforces the commitment of students to uphold honesty, ethics, and originality in their academic work, fostering a culture of trust and integrity within the school community.*

  2. Parent Release for Child - Non-Commercial: Grants St. Paul High School permission to use your child’s image, name, voice and/or work for noncommercial purposes: periodic newsletters, and other promotional materials for information and recruitment purposes.*

  3. Medication Permission Form: grants authorization to administer prescribed medications to a student during school hours, ensuring their health and well-being while adhering to proper medical protocols.*

  4. Parental Permission for Access to the Internet: As technology is an integral part of education, we encourage you and your child to review and sign this agreement. It outlines guidelines for responsible and safe technology use at school and promotes digital citizenship.*

  5. Electronic Communication Agreement Without Stationary: Establishes guidelines and consent for the use of electronic communication channels between the school, parents, and students, promoting efficient and secure information exchange.*

  6. Acceptance of Parent-Student Handbook 23-24: Accepting the parent-student handbook acknowledges understanding and agreement with the school's policies, procedures, and expectations, fostering a collaborative partnership between the school, parents, and students.*

  7. Emergency Card 2023-2024: A comprehensive document containing vital information about the child, including contact details, medical history, and emergency contacts, ensuring their safety and well-being at school.*

Completing these parent forms early in the school year allows us to focus on the most important aspect of our work – providing an exceptional and nurturing learning environment for your child.
