Athletic Forms For Athletes
Emergency and Physical Form: All athletes must have a valid physical and emergency card completed in the front office prior to participating in any sport on campus.
Athlete’s Code of Ethics: Important document provided by the CIF Southern Section. Every athlete must read, agree to uphold, sign it and return it to the Athletic Department.
Varsity Letter Request: This document must be filled out and signed by the varsity head coach of the sport the letter was earned. Each program has a set of specific requirements that must be accomplished in order to earn a varsity letter.
