Athletic Forms

Athlete’s Code of Ethics:  Important document provided by the CIF Southern Section.  Every Athlete must read, agree to uphold, sign and return to the Athletic Department.

Varsity Letter Request: This document must be filled out and signed by the varsity head coach of the sport the letter was earned.  Each program has a set of specific requirements that must be accomplished in order to earn a varsity letter.

Physical Forms: Each athlete must have a current physical form filled out and on file prior to participation in any sport.  Click the link above to download and print a copy of the physical form.
